Welcome and Let’s Get Started:


Thank you for taking the steps to sign up for our  Nutrition Therapy One-On-One sessions. I am so thrilled to be a part of  your journey to wellness. Before we meet, if you please complete all of the recommended forms so we can capitalize on our time together for our first session. If you are using insurance via Fay Nutrition, you can find your co-pay via through the Fay Nutrition Book button. Please feel free to email me at info@lyniseperrynutrition.com, if you have any questions. You can upload any recent labs into the document section of your portal. Once we’ve met to create your personalized plan for your health and wellness needs, I'll follow up with the support you need, including:

• Sample meal and  basic supplement plans with other resources to start, which  you will receive within 3-5 days via your Healthie or Practice Better portal.  It may be best to download the app, so you’ll get notification on both your phone and email. 

• Nutrition and lifestyle recommendations in the form of a protocol or care plan that includes nutrition and wellness education to support you in between our biweekly follow-up sessions. 

• Access to Food and Lifestyle Journal to track meals, mood, and activity along with a chat to communicate briefly in between sessions via your Healthie or  Practice Better Apps

ABOUT FUNCTIONAL NUTRITION ADD-ONS:

To enhance our time together, so that you are successful in meeting your health and wellness goals, I would encourage you to take advantage of my Functional Nutrition services where I’ll employ Functional Nutrition modalities, evaluations, lab analysis with reports, and weekly customized meal plans to support core imbalances that get to the root cause of your wellness challenges.  If you have already signed up for our Functional Nutrition Packages, you’ve already invested in these services, but if you are using your insurance, unfortunately health insurance companies do not reimburse for these services. If this is you, there is great news!  You can include weekly meal planning and functional blood chemistry analysis or a full  functional nutrition package as an add-on to your one-on-one sessions with me. I have included the links in this welcome letter for you to learn more and sign up.   Now you're equipped with specific steps to take and insights to help you reach your  goals! 

Just remember:  

   Lifestyle transformation can be tough. You might even question why you started in  the first place-that's totally normal. Soon, the changes you're making will become  a part of your routine. Give yourself some grace and have patience. You have all the support you need. Feeling your best is just around the corner!
