What if you are not in network with my insurance?

If I am out-of-network with your specific insurance plan, visits may still be covered. Upon request, I can provide you with a superbill to submit to your insurance for possible reimbursement. Payment for services will be due at the time of your visit.

Call your insurance company prior to your visit to ask the following questions…….

  • Is nutrition counseling covered under my plan (CPT codes 97802,97803,97804)

  • Are there any restricted diagnoses? Note: If you have an eating disorder, diabetes, lipid disorder( like high triglycerides or cholesterol), kidney disease, or a BMI greater than 25, please specify this when asking this question. Is Z71.3- preventative nutrition counseling covered?

  • Do I need a referral or prior authorization for my visits to be covered? (Note: HMO plans will require a prior authorization). All clients will need their physician to provide us with a referral with their medical diagnosis or Z71.3. Physicians can either use their own referral form or use mine HERE.

  • Is nutrition counseling covered when provided via telehealth? If so, is there an end date to telehealth sessions being covered under my plan?

  • How many visits are covered?

  • Do I have to meet my deductible before insurance pays anything towards my visit?

  • Will I have a copay or coinsurance?

  • Is Lynise Perry, LDN or Lynise Perry Nutrition ( EIN: 83-1788264 and NPI:1770065922 or NPI: 1770138091) covered as in-network under my plan? If not, what are my out-of-network benefits?

  • Write down this information along with the date, time, and reference number for the call for your records. If possible, you may want to ask them to send you the information they provide you in writing. We will need this information and your Nutrition Verification Form completed prior to your first appointment. This helps to ensure that you get the most out of your insurance benefits and avoid any surprise bills!

  • Insurance coverage varies greatly between different insurance companies and plans. If your insurance declines your visit(s) for any reason, you are responsible for the full amount billed.

  • PLEASE NOTE: When you schedule your initial appointment, you will be asked to provide a credit card that Lynise Perry Nutrition, LLC. will keep on file. In the event your claim gets denied by your insurance company (or you have an outstanding deductible or copay) your credit card will be charged for the visit. This initial and follow-up appointments are $200 and $180.00 for 60 minute visits, respectively.
